Department of Respiratory Medicine is one of the key departments of Maharaja Agrasen Hospital. Respiratory Medicine as a sub-specialty discipline in medicine is an integral and essential component of any healthcare system. Our unit is a recognized centre of excellence in India, providing international standards of patient care with compassion at affordable cost.

It is staffed with excellent team comprising of expert Pulmonologists with high professional titles, Respiratory therapists & well-trained professional nurses. Respiratory Department makes common diseases, frequently occurring diseases and critical diseases such as bronchial asthma, Chronic Obstructive Pulmonary Disease (COPD), pulmonary infection, respiratory failure, sleep apnea syndrome, Sarcoidosis, Lung Cancers, rare and persistent respiratory diseases and extreme respiratory system failure and others as the priority of clinical work.

The Bronchoscopy & Interventional Pulmonology Lab

It has two latest video Bronchoscopes with Narrow band Imaging and therapeutic facilities. All bronchoscopic procedures like Biopsies, Brushings, TBLB, TBNA & BAL are performed. Newer therapeutic options available for patient care include Endobronchial Ultrasound for diagnosis of Mediastinal Pathologies (EBUS), Rigid Bronchoscopy, APC/Laser ablation & Debulking of tumors and lysis of fibrotic tracheal stenoses, Silicone and metallic stent placements, Endobronchial Valve Placements, Intrabronchial Glue therapy. Diagnostic Thoracoscopy & Thorocoscopic procedures such as talc pleurodesis and Pleur-X catheter insertion for malignant pleural effusion are also provided apart from routine Thoracocentesis, Pleural Biopsies & Pleural Ultrasound. Basic Monitoring and resuscitation facilities are available in Bronchoscopy suite itself. Bronchoscopy procedures are also routinely done in various intensive care units of the hospital as and when need arises. The department plans to start Bronchial thermoplasty for treatment of Asthma in near future.

The Pulmonary Function Laboratory

Equipped with advanced technology with facilities to do spirometry, lung volumes diffusion and advanced Cardio pulmonary exercise Systems the lab is one of the busiest in Delhi. Comprehensive lung function testing for patients of Asthma, COPD, Occupational Lung Diseases and ILD is done. Smokers are motivated to get their lung function as a part of Smoking Cessation Clinic run by the department.

Sleep Disorder Lab

The Sleep Disorders service provides state-of-the-art evaluation and management of patients with suspected or documented sleep disorders, and medical conditions related to sleep disturbance. Obstructive Sleep Apnoea is a disease which is common still goes undiagnosed especially in patients with Cardiovascular diseases Hypertension & Diabetes and the best way is to perform sleep studies and prevent any serious complications including sudden death. We evaluate all such patients with our state-of-the-art inpatient sleep laboratory facility & provide comprehensive multidisciplinary approach including assessing therapeutic interventions such as non-invasive positive pressure ventilation (including CPAP or BiPAP).

Allergy Lab

We do all allergy tests including prick tests, Intradermal tests & serum testing to identify allergic disorders. This is followed by comprehensive programme to provide immunotherapy & use of newer biologicals like anti IgE treatment.

Pulmonary rehabilitation Services

A dedicated team of pulmonary physiotherapists is providing rehabilitation services for quick recovery and support to lead a normal life to all respiratory disorder patients.

Endobronchial Ultrasound

Endobronchial Ultrasound (EBUS) is a minimally invasive approach to the sampling of difficult to access lymph nodes or central masses in the chest. Lymph nodes as small as 10-15 mm can be sampled and the technique has broad applications including the diagnosis and staging of cancers of the lung and other cancers that are suspected of spreading to the lymph nodes in the chest. It is also useful in the sampling of lymph nodes in lymphoma and non-cancerous conditions such as sarcoidosis or tuberculosis which are always difficult to diagnose in a country like India.

EBUS involves the patient being put under Local Anaesthesis or conscious intravenous sedation; a thin flexible telescope (bronchoscope) is inserted via the patient’s mouth/nose into the lungs. Images of the region between the two lungs (the mediastinum) are obtained using an ultrasound probe attached to the bronchoscope. The aim of the procedure is to help diagnose diseases that cause enlarged lymph nodes in the chest, and for patients with lung cancer, to establish whether the disease has spread beyond the lung. It allows physicians to perform the Transbronchial Needle Aspiration (TBNA) procedure to obtain tissue or fluid samples from the lungs and surrounding lymph nodes under direct vision.

Benefits

Minimally invasive procedure that can be performed as a day case, using intravenous sedation or under local anaesthesia. The procedure is very well tolerated and there are no reported significant complications. The improved images from the ultrasound allow the physician to view difficult-to-reach areas and to access more lymph nodes for biopsy. Current evidence suggests that EBUS is a sensitive and specific as surgical mediastinoscopy in staging non small call lung cancer.

Dr Bharat Gopal is a Senior Consultant & Coordinator in Dept. of Respiratory Medicine at Maharaja Agrasen Hospital, Punjabi Bagh, New Delhi. He received his MBBS degree from the prestigious Maulana Azad Medical College, New Delhi, M.D. degree in Respiratory Medicine at Vallabhbai Patel Chest Institute, New Delhi, DNB degree and Membership of National Academy of Medical Sciences (MNAMS) for his academic excellence from National Board of Examination. Always striving for excellence in the field of pulmonology Dr Gopal went abroad and completed his training in Advanced Bronchoscopy at Singapore General Hospital, Sleep Disorders training at Brussels & Fellowship in Medical Thoracoscopy at Marseille, France. In recognition of his academic forte, he was one of the first Indian doctors to be awarded a scholarship by the European Respiratory Society in Interventional Pulmonology at Ancona, Italy.
